A Resource Guide for End of Life Services in Massachusetts has been developed by the Massachusetts
Commission on End of Life Care, with the goal of making it
easier for residents of the Commonwealth to find needed
services. The information in the Resource Guide will
be used by individuals with life-threatening illnesses, family
members and medical professionals who care for them
.

In order to qualify for a listing in the guide, an agency, organization or individual must provide some type of end of life direct care service, including medical, personal care, psychosocial, legal or educational services.

If your organization or agency is interested in being listed in the guide, please fill out and submit one of the surveys below.

Service Provider Surveys
It is still possible for additional services to be listed. Please
identify any services your organization or agency offers that are specifically geared towards people living with life-threatening illnesses and their caregivers. Your response to the survey will also assist the Commission in identifying gaps in services and working to fill those gaps.
