Description of resource: Gardner Visiting Nursing Association is a not-for-profit home health agency serving the residents of Gardner and 15 surrounding communities since 1906. In addition to providing traditional home health services, we also provide a myriad of services including an Assisted Living Program, three Adult Day Health Centers, My Home Connection – a geriatric case management program, and numerous community outreach programs. The Gardner VNA also has a certified Hospice program that provides comprehensive and compassionate end of life care for patients with terminal illness. Hospice enhances the quality of life while focusing on comfort and support for our patients and their caregivers. Contact:
