Description of resource: BIDMC Palliative Care Consult service provides comprehensive, interdisciplinary care for patients with life threatening illness and their families. The service addresses issues of pain and symptom management, end-of-life care issues such as health care proxy, advance care directives, and discussions to determine plan of care. The consult service works in collaboration with case management and social service to achieve seamless transitions from the hospital to the home, skilled nursing facility, rehab or hospice. Bereavement support is also available.
