Description of resource: Fallon Community Health Plan (FCHP) is proud to be a leader in the senior health care marketplace. In addition to our successful Fallon Senior Plan, a comprehensive Medicare Advantage plan, we offer many programs, services and resources that enrich the quality of life of our members and seniors in our community.

Fallon Community Health Plan sponsors Summit ElderCare (SE). SE is a Program of All-inclusive Care for the Elderly (PACE) that gives older adults and their caregivers an innovative alternative to nursing home care. Participants have access to most medical services at an SEC site, while keeping the independence of living in their own homes, in their own communities. There are currently two sites located in Worcester and Charlton. Transportation is provided.  Individualized care is provided by a team of geriatric professionals who work together with participants and caregivers to address each individual's specific needs. Summit ElderCare is the only program of its kind in Central Massachusetts. Website, www.summiteldercare.org.
